Sponsored by The Hong Kong Jockey Club Charities Trust, YWCA Jockey Club Y Care Elderly Centre launches Jockey Club “Happy Home Community” Rehabilitation Program for the needy, so that they can enjoy comprehensive rehabilitation training and support services even when they are back home.

While elders who lack support will be given priority, the following patients are eligible for the service:

• suffered from stroke, fracture, chronic obstructive pulmonary disease, nervous system diseases, chronic diseases etc. and are recently discharged from the hospital;
• have the potentials to recover; and
• need transitional care and support.

The Association’s physiotherapists, occupational therapists and nurses will customise rehabilitation plan for each participant based on their assessment results. Advanced facilities will help patients with different needs. For example, virtual reality system can enhance joints’ flexibility, and Thera-Trainer balo can improve muscular endurance and balance control.

Moreover, the centre offers home-training programme for the patients. Our community rehabilitation trainees will first demonstrate how to use the online system at the patients’ home. They will then conduct regular home visits to follow up on the case. Participants can communicate with their therapists by uploading their training videos or leaving comments onto the online system. Meanwhile, our therapists can evaluate the progress of the patients and provide adjustment on their training plan if necessary.
