The population in Hong Kong is aging. Many elderly suffer from chronic diseases without knowing, and the condition of some get worse because of poor health monitoring.HKYWCA has been in partnership with "Jockey Club Community eHealth Care Project – Stay with Health without distance" for three years, as a means of improving the health monitoring and management of patients with Triple H, hyperlipidemia, hyperglycemia and hypertension.

Triple H patients were invited to visit one of the five elderly centers of the Association twice a week to measure their blood pressure, glycemia, body weight and body temperature. Health data will be transferred to the nurse team by cloud technology for proactive monitoring and follow up immediately. "Once there was a patient having abnormal blood pressure because he has forgotten to take antihypertensive drugs. He received a call from the nurse reminding him to take medicines, before he even left our centre," one of the social workers said. Meanwhile, over 4,000 people participated in different kinds of lectures and workshops under the concept of ‘Yellow Emperor's Canon of Medicine’ to improve their health knowledge.

This 3-year project ended last month with a ceremony, in which 450 active participants were invited to join. Many of them said they have better  blood pressure and hyperglycemia. 

With the remarkable results of the previous project, the Hong Kong Jockey Club Charities Trust earmarks new funding to support the project for the next 3 years. It will be launched in the first half of 2020.
